Medanta-The Medicity - Pioneering Cement-less Total Knee Replacement performed


Gurugram, February 23, 2017: Medanta - The Medicity successfully performed another cement-less Total Knee Replacement (TKR) with the help of Titanium Nitride (TiNi) implant. It was the second case in India as well as at Medanta.

Dr. SKS Marya, Chairman, Institute of Bone & Joint, Medanta - The Medicity planned and executed a unilateral knee replacement for the first time for a 63 year-old, Ishrat Umar, who was hypersensitive to metals and allergic to standard implants. Her treatment was successful with no adverse effects observed even two months post the surgery.

Basis the postoperative outcomes, a similar bilateral (both knees) total knee replacement has been performed on a 52 years old patient from Afghanistan, Mohd Qasim Alfaqir. The patient desired for maximum movement of knees, to be able to perform his religious rituals and daily routines with ease.

Dr. Marya, who led both the surgeries at Medanta Hospital said, "the TiNi implant provides an advantage of cement-less fixation, a coating with layers of porocoat titanium nitride ceramic, which is a structural material complementing that of a human bone. With the passage of time, the human bone grows into this surface. It also provides a mobile bearing surface which gives additional movement to the knee and reduces the rate at which the joints start wearing out. Hence, this particular implant is able to deliver a functional advantage along with an added advantage of increased longevity”.

Titanium nitride (TiN) sometimes known as tinite, is an extremely hard ceramic material, often used as a coating on titanium alloys, steel, carbide, and aluminium components to improve the substrate’s surface properties. Applied as a thin coating, TiN is used to harden and protect cutting and sliding surfaces, for decorative purposes (due to its gold appearance), and as a non-toxic exterior for medical implants. In most applications a coating of less than 5 micrometers is applied.

The major advantage of cement-less surgery is that the revision of joints which happens nearly after 20-25 years, can be done smoothly as there is no actual bone loss. Following are other advantages of this cement-less implant:

- Those who have an active lifestyle and desire a higher degree of mobility from the implants

With the passage of time, a new bone grows into the implant surface (bio-fixation).
During operation, it takes 10-15 minutes to set cement on either side. Thereby, in a cement-less knee replacement procedure, one saves a minimum of 20 minutes of operating time.
The implant is mechanically fixed and not grouted using cement making revision surgeries at a later date easier. It is ideal for relatively younger patients with strong bones.
According to Dr. Marya, the principal of fixation depends on the precision of a high-quality surgery. The initial stability of implant is achieved by mechanical fixation that can be observed over the next 6-8 weeks as the bone grows over the TiNi implant and post this it provides a biological fixation.

Both these patients operated at Medanta are now able to walk and carry on with their daily routines with ease.

February 15th 2017, Gurugram: India's first 3D printed artificial vertebrae gives a new life to a 32-year-old Spinal Tuberculosis (TB) patient. The Complex Spinal Reconstruction was recently conducted by Spine surgeons that helped her walk once again.

The team of surgeons led by Dr. V Anand Naik, Senior Consultant Spine Surgery, Bone & Joint Institute from Medanta - The Medicity, Gurugram replaced the damaged vertebrae with a 3D printed titanium vertebrae - to bridge the gap between the first and fourth cervical vertebrae.

As a pending history of infertility, the patient suffered tuberculosis due to high intake of steroid (by quakes) which lowered her immunity to a level where the patient developed TB at 10 different vertebrae in the spine. The first, second and third cervical vertebrae were severely damaged up to an extent that there was no skeletal support available between skull and lower cervical spine (there was disconnect between skull and lower part of the spine) due to which the head was sliding forward and curved in such a way which was causing obstruction to the spinal cord which was causing progressive weakness in all the limbs – inching towards quadriplegia; compression of the respiratory nerves and eventually death.

This 10-hour long surgery is the first of its kind for reconstruction with 3D printed vertebrae in India and probably third in the World, following similar surgeries that were conducted in China and Australia for different parts of the spine.

Dr. V Anand Naik, said, "it was a very complex surgery and the patient's condition was deteriorating each day. It would not have been possible to do it without a 3D printing technology."

"It is comparatively an effective method of surgery since everything is planned before, and also a new dimension to complex spine reconstruction in difficult anatomy and complex Tumors and infections," he added.

Shedding more light on the treatment process, Dr Gopal Kumar, Consultant, Head & neck oncosurgeon at Medanta-The Medicity said,” The challenge for our team was to reach high into the neck without altering the position of the patient. Anterior approach, small working field, in cases such as these is not basis one’s choice but a necessity. Patient being a singer, preservation of laryngeal nerve was of prime importance. Swallowing, chewing and movement of tongue, all was at risk. After mobilisation and retraction of larynx and oesophagus space was created for Dr. Anand to work and perform the implant.”

Dr. Naik and his team used an intricate computer software to plan every detail of the operation. The titanium cage was customised according to the patient’s original spine. The high-resolution CT & MRI scans of the patient spine were uploaded on the software and a dummy of the patient’s spine was 3D printed to measure the gaps & surgical resection between the first and the fourth cervical vertebrae. Finally, the 3D titanium implant was printed, which was to be placed in the body. The printed 3D titanium vertebrae were further tested for biomechanics and stress risers post receiving inputs from design team in India, Sweden and USA.  The standard time to plan and perform the surgery is between three to four weeks. Dr. Naik (Spine) & Dr. Kumar (Head & neck) at Medanta The Medicity, effectively planned the approach and executed the complex upper cervical spine surgical approach and reconstruction, within a week, in a much lesser time citing the emergency of the case.

The patient who is a teacher by profession is recovering fast with newly reconstructed cervical vertebrae. She has recovered from all her weaknesses & radiculopathies in Upper and Lower limbs within 4 days of surgery and is now able to walk. She will be able to lead an independent pain & risk-free life in 2 weeks.

Dr. Naik and his team are now looking forward to a long term result and a decent independent lifestyle for the patient.

57th Annual Conference of Indian Society of Gastroenterology being organised by Medanta

New Delhi, November 22, 2016: The Government of India’s extensive programme of eradicating Tuberculosis is being supplemented by “Mission TB Free Haryana”, an initiative by Medanta - The Medicity. The mission was launched on November 20, 2015 by Medanta and has completed one year of operation. An event, attended by Shri Anil Vij, the Hon’ble Minister of Health, Govt. of Haryana, commemorated the efforts over the last one year in supporting the Government’s initiative of tackling the plague of Tuberculosis. The event was preceded by a “Planning Meeting” between the Government’s District TB officers and Medanta’s Doctors, to chart out the way forward for the mission.

Hon'ble Minister of Health, Shri Anil Vij and Dr Naresh Trehan launched 5 vans for Mission TB Free Haryana today at Medanta

Under the Mission TB Free Haryana, Medanta renders the medical expertise and equipment for secondary screening of Tuberculosis. Chest X-Rays will be used to detect TB among patients who may erroneously have shown to be Sputum Negative for TB. (Sputum test is the most common test for Tuberculosis in the Districts). The mission covers the 21 districts of Haryana. In its first phase, the mission included Rewari, Gurgaon, Mewat, Palwal and Jhajjhar, where the vans equipped with chest X-Ray machines and other equipment were deployed to work in tandem with the Government’s Primary and Secondary Health Centres to ensure more effective TB Diagnosis.


The mission is spearheaded by Dr. Bornali Datta of the Department of Respiratory and Sleep Medicine, Medanta under the aegis of “Call to action for a TB-Free India”, by the Government of India.


With the completion of a year, the event highlighted the results of the Mission where approx. 3000 number of people were treated for TB and also announced the launch of five new MISSION TB FREE HARYANA Vans. 

Stressing on the need for improved case findings for TB, Naresh Trehan, CMD - Medanta said, "More than 50% of patients with pulmonary TB go undetected in India currently. The rise of the drug-resistant strain has made the situation even worse. Social stigma, lack of awareness and inadequate medical facilities further aggravate the problem. This screening programme under TB Free Haryana initiative aims to guide patients about their status and encourage them to start treatment for TB as soon as possible. This will help reduce the spread of the disease."


The event witnessed dignitaries like - Hon’ble Minister of Health - Shri Anil Vij, Govt of Haryana, USAID/India Mission Director from US Embassy – Mr. Jonathan Addleton, Dr. David Ford from Scarborough, UK, Dr. T. C. Tanwar, District TB Officer and Dr. Sunil Khaparde, Additional DDG - TB, among others.

To bridge this gap, Indian Medical Association, Gurgaon (IMA) in association with Medanta - The Medicity, MOHAN Foundation (Multi Organ Harvesting Aid Network) and Association of Physicians of India, Gurgaon (API) today celebrated The Organ Donation Day as part of their ongoing efforts to increase awareness around this cause and thereby, enrolment.

Celebrate Organ Donation Day by Pledging your Organs


The Organ Donation Day is celebrated every year to honor those who have given the gift of life through this noble act and to educate others, on the significance and importance for those in need. As a fact, each person can save the lives of up to 7 individuals and can enhance the lives of over 50 people by way of tissue donation.

Highlighting the importance of Organ Donation, Dr. A. S. Soin, Chairman Liver Transplant & Regenerative Medicine, Medanta – The Medicity said, “Around 2 lac patients with liver failure and 50,000 patients with liver cancer die in India every year. At least 15% of these patients can be saved with liver transplants, which now has a success rate of 95%. Of nearly 40,000 needed, currently only 1,600 liver transplants (75% from live donors and 25% from deceased donors) are done in India annually. This number could easily become four times if the deceased donation (from brain dead donors) rates were to become even 2/million population from the current 0.4/m. In the USA, the donation rate is 25/ million.”

In an effort to bridge this gap, IMA Gurgaon in association with Medanta - The Medicity, MOHAN Foundation and API Gurgaon will also be hosting a CME to facilitate the donors and recipients who volunteered to donate their or their loved one’s organs to save the life of others.

Supporting this cause Dr. Ashok Taneja, Member Human Organs Transplantation Board, Govt. of Haryana said, “Presence of hospitals like Medanta performing kidney, liver and heart transplant ensures that Haryana is fast emerging as a transplant state thereby, giving patients with end stage organ failure a chance of a new life. The Haryana Government is committed towards this cause. This CME is one of the steps in that direction as educating the medical fraternity is important.”

Celebrating this cause Pallavi Kumar, Executive Director, MOHAN Foundation said, “Though there has been a threefold growth in the organ donation numbers over the years, from 196 donors in 2012 to 528 last year, India still has a long way to go before it meets the current requirement. Also, this development has been skewed, with only a selected few states like Tamil Nadu, Kerala, Maharashtra, Karnataka and Andhra Pradesh/Telangana making a progress and the rest of the country showing negligible organ donation and transplant activity. This scattered behaviour is only benefitting a few sections of the population. We therefore, need all the stakeholders to play a stronger role - be it the government at the Centre or at the state level, the hospitals both private and government or civil society organisations – to make this a mass movement. We need to educate not just the public but also the medical fraternity to create a conducive environment for families to consent for organ donation.”
